About the form
The OHF29 Job Hazard Evaluation Form must be completed and a copy forwarded to Occupational Health where a position being recruited to involves exposure to job hazards that require the individual to be registered on a health surveillance programme. Once a new employee has been offered the role, Occupational Health must be asked to arrange an appointment for him/her to undergo a health assessment.
