About the form

The OHF29 Job Hazard Evaluation Form must be sent to the line manager / supervisor from the recruiting manager where a position being recruited to involves exposure to job hazards identified on the form.

The form must also be completed by the line manager / supervisor with the employee when a change in job role involves exposure to job hazards identified on the form.

Using the form

The line manager / supervisor must complete the form with the employee and all accompanying information and relevant risk assessments must be sent as per the instructions. If not received this will result in the form being returned to the sender and Occupational Health will not be able to proceed with individuals evaluation.
