HR19 Application form for DBS check

About the form

This document must be completed and submitted with each DBS check (formerly known as CRB check) application form. It is intended to:

  • Help departments/institutions understand which roles are eligible for DBS checks.
  • Ensure that the University only requests DBS checks where it has a legal entitlement to do so.
  • Prompt departments/institutions to undertake a risk assessment where they want a new employee to start work if a DBS check is not completed before the individual's intended start date.

DBS applications will not be processed until a completed HR19 form has been received.

Using the form

The document contains instructions about how to complete the form. Please ensure that you follow these so that:

  • It is clear from the form how the role in question is eligible in law for a DBS check.
  • The risk assessment has been completed where you would want the new employee to start work if a DBS check is not completed before the individual's intended start date.

HR19A DBS Status Check Request

About the form

This document must be completed and submitted instead of an HR19 form where an applicant for a DBS check (formerly known as CRB check) has confirmed to you that they have subscribed to the DBS Update Service. The form enables the HR Division to complete an instant, online check (known as a status check) to confirm that the applicant’s previous DBS check is still up-to-date rather than having to complete a new check.

It is intended to ensure that the University only undertakes a DBS Status Check where it has a legal entitlement to do so.

The document contains instructions about how to complete the form. Please ensure that you follow these so that it is clear from the form how the role in question is eligible in law for a DBS check and that the requirements for a status check to be used have been fulfilled.
