- RP2A: Request for University Officer to Continue Working Beyond the Retirement Age
- RP2B: Details of Funding
- Curriculum Vitae
- Minutes of the relevant body for example, the Needs Committee (or equivalent)
- RP4: Application For Flexible Retirement (where appropriate)
