The formal and final probation assessment forms have been designed to facilitate the documentation of formal assessments of progress during the probationary period as well as the final overall assessment towards the end of the probationary period. Note that the completed documentation should be retained by the department.
Departments and probationers are also able to use Form HR60 to record key objectives at the outset of probation.
